Referrals & Bookings
At London Diabetes Foot Clinic, patients can access our services through self-referral, healthcare provider referral, or online booking through our secure Jane App portal. We welcome referrals from physicians, nurse practitioners, home care providers, hospitals, long-term care homes, community agencies, and patients themselves.
Book an Appointment
Many appointments with our nursing team can be booked online through our secure Jane App portal. If you are unsure which appointment to book, please contact us and we will be happy to guide you.
Patients may book appointments with Jennifer Dresser, RN, BA, BScN, MClSc-WH (Advanced Practice Foot & Wound Care Nurse) and Lynda Cottrill, RN, BScN, NSWOC, WOCC(C), IIWCC (Advanced Foot & Wound Specialist Nurse) for eligible services.
Some appointments, including custom orthotic assessments with Kim Trotechaud-Allen (certified orthotist) combined wound care and offloading appointments, and pro bono appointments, require direct scheduling through the clinic.
Self-Referrals Welcome
Patients do not require a referral to access services at London Diabetes Foot Clinic. Individuals may contact the clinic directly or book eligible appointments online through Jane. If you are unsure which appointment type is appropriate, please contact us and we will be happy to help.
Orthotic and Multidisciplinary Appointments
Appointments with Certified Orthotist Kim Trotechaud-Allen, as well as coordinated appointments involving nursing assessment, wound care, debridement, and custom offloading, are scheduled directly through the clinic to ensure appropriate appointment times and coordination between team members.
