Pressure redistribution, felt offloading, orthotics, bracing, footwear modification, and ulcer prevention.
Coordinated Care
Collaboration with family physicians and nurse practitioners, Ontario Health at Home and other providers when additional care is needed.
Who We Help
We provide multidisciplinary assessment, wound care, offloading, orthotic support and ongoing follow-up for people at risk of foot wounds, delayed healing, mobility complications, and limb loss. Our clinic commonly supports individuals experiencing:
- Diabetic foot ulcers
- Neuropathy and loss of protective sensation
- Charcot foot deformities
- Recurrent wounds and callus formation
- High-risk feet requiring ongoing monitoring

Our Approach
We believe effective foot, wound and limb preservation care works best when advanced wound care, offloading, orthotic expertise, education, and close follow-up are connected in one collaborative setting. Our approach focuses on prevention, early intervention, individualized care, and helping patients navigate available supports and services whenever possible.
We work collaboratively with patients, family physicians, nurse practitioners, Ontario Health at Home, and other providers to support continuity of care and improve healing outcomes.

Access & Advocacy
Many advanced wound care, foot care, and orthotic services are not fully covered within Ontario's healthcare system. We help patients explore available funding pathways and believe multidisciplinary limb preservation care should be more accessible for the people who need it.
Prevention & Early Intervention
Early assessment, pressure redistribution, advanced wound care, and ongoing monitoring can help reduce complications, prevent hospitalization, and support limb preservation.
Flexible Referral Options
Patients may self-refer or be referred by a healthcare provider. Appointments may be requested online, by phone, or in person, with scheduling supported through the Jane App.
